Hip Preservation in Athletes: Maintaining Peak Performance

Hip preservation is an essential aspect of orthopedic medicine that focuses on maintaining the natural hip joint, preventing or delaying the need for hip replacement surgery. The hip joint is a ball-and-socket joint that connects the femur (thigh bone) to the pelvis and plays a crucial role in mobility and weight-bearing activities. Preservation strategies are especially critical for individuals with hip pain, dysplasia, femoroacetabular impingement (FAI), labral tears, and early-stage arthritis. A multidisciplinary approach is often required, integrating orthopedic surgery, physical therapy, pain management, and lifestyle modifications. The goal is to improve function, relieve pain, and extend the lifespan of the native hip joint.

One of the primary causes of hip pain leading to joint deterioration is femoroacetabular impingement (FAI), a condition where abnormal contact between the femoral head and the acetabulum results in cartilage and labral damage. FAI can be classified into cam, pincer, or mixed types, depending on the nature of the impingement. Cam-type FAI occurs when an abnormally shaped femoral head does not fit smoothly into the socket, causing excessive friction. Pincer-type FAI happens when the acetabulum excessively covers the femoral head, leading to pinching and damage to the surrounding structures. hip preservation -type FAI combines characteristics of both. Early diagnosis through imaging techniques such as MRI and CT scans can help in planning effective interventions to preserve the hip joint. Surgical treatment often involves hip arthroscopy or open hip preservation procedures to reshape the femoral head, correct the acetabular over-coverage, and repair labral tears. These procedures aim to restore normal joint mechanics and prevent progressive degeneration.

Developmental dysplasia of the hip (DDH) is another significant condition requiring hip preservation strategies. DDH refers to a spectrum of abnormalities where the hip socket is too shallow, leading to instability and an increased risk of early arthritis. While DDH is often diagnosed in infancy, some cases remain undetected until adulthood, where symptoms such as hip pain and instability become apparent. Periacetabular osteotomy (PAO) is a common surgical procedure used to treat symptomatic DDH in adolescents and young adults. PAO involves cutting and repositioning the acetabulum to provide better coverage of the femoral head, thereby reducing joint stress and improving stability. When performed at the right stage of the disease, PAO can significantly delay or even prevent the need for total hip replacement.

Hip labral tears, often associated with FAI and DDH, also play a significant role in hip joint deterioration. The labrum is a ring of cartilage that lines the acetabulum, providing stability and cushioning to the joint. When torn, it can lead to pain, mechanical symptoms such as clicking or locking, and increased stress on the cartilage. Hip arthroscopy is the preferred method for treating labral tears, involving either debridement (removal of the damaged tissue) or labral repair to restore function. Labral reconstruction using grafts has emerged as an option for cases where the native labrum is too damaged to be repaired. Proper treatment of labral pathology is crucial in hip preservation, as an untreated tear can accelerate joint degeneration.
